Mindful Detroit LLC Yoga Therapy Terms & Conditions

 By purchasing, scheduling, or participating in any yoga therapy service offered by Mindful Detroit LLC, you agree to be bound by these Yoga Therapy Terms (“terms”). You agree that these terms incorporate and are incorporated into the general Mindful Detroit LLC Terms and Conditions available for review at this link: www.mindfuldetroit.com/termsconditions.

Yoga Therapy Details

Yoga therapy sessions with Mindful Detroit consist of the following (details may vary depending on whether you have completed a prior yoga therapy session with Mindful Detroit):

·       Initial phone consultation lasting 15 minutes during which the therapist describes yoga therapy services and address potential client questions.

·       Intake form completed by the client online and emailed back to the therapist. The intake will take approximately 20-30 minutes to complete.

·       One 90-minute Initial Assessment, which includes review of the intake, various assessments, and starting to create a treatment plan including doing activities together in session and assigning some activities for the client to do at home.

·       One 60-minute Treatment Planning Session (the first follow-up session), which includes creation of a treatment plan or adding to what was started in Initial Assessment, including doing activities together in session. The treatment plan is created jointly by the client and therapist.

·       Treatment Plan activities done by the client at home: The type of activities, the frequency, length, and number are determined by the client and the therapist. Activities may include asana (movements), pranayama (breath-work), relaxation, meditation, mudra (hand gestures), bandha (energy locks), mantra (sacred sounds), mindfulness, bhavana (imagery), sankalpa (affirmation/intention), yoga nidra (guided rest), chakra exploration (energy centers), yogic lifestyle, yoga philosophy, and other practices in the yoga tradition.

·       Sixty-minute Follow-up Sessions (number determined by client and therapist), which include reviewing treatment plan activities done at home, any barriers to completion, doing existing or new treatment plan activities together in session, and possible follow-up on postural or joint mobility assessments to track progress.

Scheduling Services & Client/Therapist Communication

·       After the initial phone conversation, the potential client should indicate their decision to proceed with yoga therapy services to the therapist via phone or email. If the client decides to proceed, the client will book at date for the initial assessment at least 1 week out. The client will pay for services and the therapist will email the client the intake form.

·       A minimum of 3 days is needed between the therapist receiving the completed intake and the initial assessment. If the intake is received later than planned, the initial assessment date may need to be pushed back.

·       One week is recommended between the initial assessment and first follow-up session.

·       Follow-up Sessions occur weekly or biweekly and 5 session packages expire 11 weeks from the first session and 10 session packages expire 22 weeks from the first session. This is to maintain consistent practice and allow adequate time for the client to implement the treatment plan. Follow-up sessions can be scheduled via the Mindful Detroit website.

·       Mindful Detroit will be available to the client by email between sessions (typical response time is 24-48 hours, Monday-Friday, 8am-5pm) and can check in with the client periodically (frequency determined by client) to encourage treatment plan adherence.

 Service Location

Mindful Detroit yoga therapy services can be provided at the following locations:

·       The client’s home with masks (depending on COVID rates) and proof of vaccination. The yoga therapist is available to travel up to 20 miles from central Detroit to conduct services in a client’s home. A private space where the client and therapist can talk confidentially and without interruption is requested. A cat-free room is also requested due to therapist allergies. Sanitized props can be provided during sessions although it is best if the client has their own.

·       Virtually via Zoom. The therapist will email the client a Zoom meeting link and password. The client will need to download Zoom on their device. There is no cost to the client to use Zoom. A private space where the client and therapist can talk confidentially and without interruption is requested. The device screen should be adjusted so that therapist and client can both see each other’s full bodies for movement and assessment purposes.

o   For virtual services, the therapist may require a photo of the client’s full body from the front and the right side, at least 48 hours prior to the initial assessment. This will allow the therapist to use a posture assessment app to assist with postural assessment. If the client is not comfortable with this they should discuss any concerns and possible alternatives with the therapist prior to services.

Payment

 Payment is due at time of scheduling.  Payment can be made via debit or credit card on Mindful Detroit’s online scheduling page.

 Packages have a split payment option to pay a 50% deposit at time of purchase. The remaining balance is due by the third session and can be paid through the scheduling page.

Occasionally HSAs (Health Savings Accounts) or FSAs (Flexible Spending Accounts) will cover yoga therapy. This may require a referral from a licensed healthcare provider.

·       It is the client’s responsibility to obtain any referral requested by the HSA or FSA.

·       Mindful Detroit will provide receipts of payment and a summary of services provided, which the client can submit to their HSA or FSA if interested.

·       It is the responsibility of the client to obtain reimbursement through their HSA or FSA.

Cancellation Policy

 ·       If the client would like to reschedule a session, 24 hour notice is required. This will not affect payment. Clients can reschedule via the confirmation email received after initial scheduling or by emailing the yoga therapist.

·       No show/late cancel fee: If a client no shows or cancels a session with less than 24 hour notice, the client will be charged 50% of the service fee. The therapist will invoice the client and the client must pay the fee before future appointments can be scheduled.

·       If the therapist needs to reschedule a session, she will give 24 hour notice if possible (unless emergency prevents it). She will attempt to reschedule the appointment as soon as possible. This will not affect payment.

·       If the therapist is unable to continue to provide yoga therapy services, a full refund for remaining services will be given.

·       If the client would like to completely cancel services before beginning treatment and receive a full refund, a 1 week notice is required.

o   Full services completely canceled with less than 1 week notice will receive a 50% refund.

o   No refunds will be given once services have commenced. Extensions on package may be granted on a case-by-case basis.

·       For virtual sessions: If technology issues prevent the client and therapist from connecting, attempts will be made to reconnect. If this does not work, the client and therapist will reschedule the service. This will not affect payment.

·       5 session packages expire 11 weeks from the first session and 10 session packages expire 22 weeks from the first session. No refund is provided for expired sessions.

 Recording

 During in-person or virtual sessions, the therapist may record or photograph part of the session to share with you for your own personal use and reference, but will only do so if she has your prior permission to do so. If you grant the therapist permission, she will not share the recordings or photographs with anyone other than you without your express permission.
